A toddler girl died Sunday after an incident at her Wasilla child care facility on Thursday morning, according to her family members.
They say Faith Phillips, who was 19-months-old, was playing outside Illuminations Child Care on some playground equipment, when she stuck her head through a hole in the equipment while standing on a toy, slipped and strangled herself.
"There was a little window, and her head fit right through the window," said the girl's mother, 19-year-old Brittany Phillips.
According to Phillips' grandmother, Amy Thomas, the toddler was discovered by an employee, who ran to get help with CPR from another employee.
Phillips was flown to Providence Medical Center in Anchorage, where she was declared brain dead. She was pronounced dead Sunday afternoon.
"She loved to dance, sing and talk," said Faith's Step-Father Andrew Stokes. "She loved to play with our cat."
Channel 2 requested comment from the owner of Illuminations Child Care who wrote via Facebook "Our hearts sincerely go out to the family, but we have no other comment at this time."
Channel 2 tried to reach Wasilla police for confirmation of the incident but was told the investigating officer was not available until Monday.
