Alaska corrections officials say confessed serial killer Israel Keyes was mistakenly issued a razor before he committed suicide.
A report released Wednesday by the Alaska Department of Corrections also says "it appears that razor was not retrieved."
It does not say how Keyes died -- only that it was a suicide.
Keyes' body was found in his cell Dec. 2, months before he was to have gone on trial in the 2012 slaying of 18-year-old Anchorage barista Samantha Koenig.
Before his suicide, Keyes told investigators he had killed Koenig and at least seven other people across the country, including Bill and Lorraine Currier of Essex, Vt.
Investigators believe there could be more victims in Washington, Oregon and Texas.
Corrections officials didn't immediately return phone calls from the AP Wednesday.

AMacusYoung, do you think Keyes was our first high profile inmate in Alaska? There has been quite a lot going back to Charles Meach, Kirby Anthony and host of murderers that graced DOC Alaska. There is no reason to treat Keyes any different then any other inmates unless he shows signs of mental instability, getting threats or issuing threats. DOC Alaska is not a daycare. From the news report, Keyes was placed in punitive segregation unit for making a fake handcuff key. But although he is restricted in movement and locked up 23 hours per day, he is still allowed to have personal hygiene items and he is allowed to shave. The article saying that he was issued a razor by mistake is an error. (Unless there was a special post order forbidding Keyes from having a razor but there is nothing written about that!) Problem came when that housing unit officer failed to retrieve the razor from Keyes and the second officer or supervisor that suppose to double check the count miss it as well. Redundant failures! Human errors, not conspiracy result in the missing razor. Too bad Keyes couldn't wait until after he confessed all his crimes before doing his sorry butt in.
Very concerned that we have underhandedness that is about to be ignored again. Sometimes the right thing is the hard thing. The wrongdoing is clear.
